DirectLabs portal
The DirectLabs link opens an independent third-party portal. DirectLabs collects and manages the information used to create an account, order and pay for testing, obtain a requisition, and deliver results under its own notices and policies. Vitalyft Medical does not receive a customer’s results unless the customer authorizes DirectLabs to share them through a HIPAA release.
Important website privacy notice
This public website and ordinary email are not secure medical-record portals. Do not send diagnoses, medical records, laboratory results, requisitions, photographs of identifying documents, Social Security numbers, Medicaid or insurance identification numbers, payment-card information, or other sensitive health or identity information through the website contact fields or ordinary email.
If protected or clinically detailed information is needed for an approved service, Vitalyft Medical will provide instructions for an appropriate collection or communication method.
Information you choose to provide
The booking, contact, and service-intake forms ask only for limited scheduling and operational information, such as name, email address, telephone number, service category, preferred date and time, service ZIP code, location type, kit or laboratory name when relevant, and non-medical access notes.
Submitted booking and review requests are securely stored for Vitalyft Medical to review. The public forms do not ask for full service addresses, medical histories, payment-card information, identity documents, laboratory orders, or results. Vitalyft will provide an appropriate private follow-up method when additional information is required.
The optional feedback page may collect a rating, review, display name, email address, referral source, and separate permissions to feature the review or contact the submitter. Feedback is held for private review and is not published automatically.
How information is used
- To respond to questions and review requested services.
- To verify location, availability, kit, laboratory, participant, and handling requirements.
- To process an appointment request, prevent conflicting confirmed bookings, and communicate about an appointment or service request.
- To review feedback, improve service, prevent abuse, and maintain website security.
- To comply with legal, safety, accounting, and recordkeeping obligations.
Contact information submitted for a service request is not automatically enrolled in marketing. Promotional email or text messages require a separate, voluntary permission where applicable.
Payment links and outside services
Approved payment buttons lead to a secure checkout operated by an outside payment provider. Vitalyft Medical does not ask visitors to place card or bank information in its website contact form. Payment, email, hosting, laboratory, drug-testing program, carrier, and other outside service providers maintain their own systems and privacy practices. Drug-testing records and results are handled through the applicable program’s separate operating systems, not the general website form. Information is shared only as reasonably necessary for the visitor’s request, an authorized service, business operations, safety, or legal requirements.
Health information, HIPAA, and Indiana law
Vitalyft Medical protects the confidentiality of health and service information entrusted to it. When Vitalyft Medical is acting as a HIPAA covered entity or business associate for a particular service, applicable protected health information is handled under the HIPAA Privacy, Security, and Breach Notification Rules, the HITECH Act, applicable business-associate agreements, and any more protective law that applies. Those rules do not automatically apply to every visitor, website interaction, or record.
Where applicable, Vitalyft Medical limits uses, disclosures, and requests for protected health information to the minimum reasonably necessary for the permitted purpose, except when a different rule applies. Appropriate administrative, technical, and physical safeguards are used, and legally required breach notices will be provided. Vitalyft Medical does not sell protected health information.
This website policy is not a substitute for a service-specific consent, authorization, business-associate agreement, or formal Notice of Privacy Practices when one is legally required. A person may request the privacy notice or ask a privacy question by emailing care@vitalyftmedical.com or calling (317) 426-1123. Indiana confidentiality, consumer-data, and breach-notification requirements are followed when they apply.
Cookies and technical data
The website host and security services may process limited technical information such as IP address, browser and device type, requested pages, timestamps, error logs, and security events. Essential cookies or similar technologies may be used for authentication, security, fraud prevention, and basic site operation. Vitalyft Medical does not currently use this website for targeted advertising or sell website visitors’ personal information.
Retention and protection
Information is retained only as reasonably needed for the purpose collected, service administration, dispute handling, security, legal obligations, and legitimate business records. When it is no longer reasonably needed and no legal, accounting, safety, or dispute-preservation obligation applies, it is deleted or de-identified under Vitalyft Medical’s record-retention practices. Reasonable administrative, technical, and physical safeguards are used according to the nature of the information. No internet, email, or storage system can be guaranteed completely secure.
Children and caregivers
This website is written for adults arranging or considering services. A parent, legal guardian, or authorized adult should submit requests involving a minor. Children should not independently submit personal information through this site.
